Supplemental Health Plans
What are supplemental health plans?
Supplemental health plans provide peace of mind and financial protection for expenses not covered by your primary health insurance. A hospital stay, serious illness or injury may not be just a physical setback. It can also disrupt your mental, social, and financial well-being.
The following benefits offer you and your family an expanded safety net of coverage. In most cases, once you enroll, premiums will be conveniently deducted from your paycheck.
Accidental Injury Insurance
Life is unpredictable. Be prepared for life's surprises with Accident insurance. The plan pays a benefit for a covered accident and the benefit amounts depend on the type of injury and treatment received. Whether you break a bone or experience a more serious injury, you can use the money for medical bills, monthly expenses and other bills that you may have while you recover. You may elect at the time of hire or open enrollment. This benefit is administered through Lincoln Financial.
Covers wide range of common injuries and services. Benefit is based on type of injury and treatment needed.
- Pays a lump sum benefit, tax-free, cash benefit directly to you for accidental injuries.
- You may use this money however you wish to pay for prescriptions or other out-of-pocket expenses, that may not be covered by the health plan.
Critical Illness Insurance
This benefit provides cash to help pay for both medical expenses not covered by your medical plan as well as day-to-day expenses that may start to add up like rent, mortgage, car payments, etc., while you are ill.
If you or your covered dependent(s) are diagnosed with a covered illness, you get a lump-sum cash benefit, even if you receive other insurance benefits. This benefit is administered through Lincoln Financial.
- Pays money directly to you when you are diagnosed with specific critical illnesses.
- This money can help you pay for out-of-pocket medical expenses and help with other out-of-pocket costs that aren't covered by health insurance.
- Examples of critical illnesses are: heart attacks, strokes, cancer, and others (a full list is included in your application).
- Choose a benefit between $5,000 and $50,000. Spouse and child coverage also available. Spousal coverage may not exceed 100% of employee's coverage. Children may be covered up to 25% of employee amount.
- Guarantee Issue for employee and spouse is $30,000 and all child(ren) coverage is guaranteed.
- Benefit Waiting Period - None
- This plan is portable, and employee's can continue their coverage if you leave employment. You must complete an application of portability within 30 days of termination.
- Age Reduction Schedule - this plan has an age reduction schedule of 50% of the original purchase amount at age 70.
- Pre-Existing Condition clause - None
- Health Screening Benefit: For employee, spouse, and child(ren), this plan pays $50 for one health screening benefit when performed during a 12-month period for insured employees and dependents. Covered Screenings include: mammography, blood tests, chest x-rays, colonoscopy, fasting blood glucose test, bone marrow testing, flexible sigmoidoscopy, hemoccult stool analysis, pap test, PSA, stress test or ultrasound screening of breast, abdominal aortic aneurysms, carotid arteries or for cancer detection.
Hospital Indemnity Insurance
Hospital Indemnity insurance provides a fixed lump-sum payment that can help cover hospital expenses not covered by insurance, or to pay for expenses while you, your spouse, and/or dependents are in the hospital. This benefit is administered through Lincoln Financial.
- Hospital Room & Board - $100 per day; limited to 180 days per coverage year
- Hospital Admission - $1,000 per coverage year
- On-Call Travel Assistance - Included
- Guarantee Issue - no medical questions
- No pre-existing conditions exclusion
- Mental and nervous and substance abuse treated the same as any other hospital admission
- No deductibles
- Coverage offered on a voluntary basis
- Continuation of Coverage is available. You must notify Reliance Standard within 31 days of termination if you wish to continue the coverage.
Group Whole Life Insurance
Group Whole Life Insurance, also known as permanent life insurance, provides employees with lifelong coverage and valuable financial security. Unlike term life insurance, which only lasts for a set number of years, whole life insurance stays in effect as long as premiums are paid. In addition to the guaranteed death benefit, this type of policy also builds cash value over time—a savings component that grows tax-deferred and can be accessed through loans or withdrawals to help with future financial needs. This combination of protection and cash value accumulation makes group whole life insurance a strong option for employees who want lasting coverage along with a financial resource they can use during their lifetime. This benefit is administered through Atlantic American.
- Group Rates: Available through payroll deductions for you, your spouse, and your children.
- Guarantees Premium: The cost of your life insurance policy will never increase as long as premiums are paid.
- Guarantee Cash Value: The cash value at age 65 shown at the time of purchase is guaranteed provided your coverage remains in force.
- Guaranteed Issue: Eligible employees actively at work can purchase this coverage up to the established limit, regardless of past or present health conditions.
- Guaranteed Additional Purchase: By purchasing even a minimum amount of coverage, you are guaranteed the right to purchase any remaining portion of the guaranteed issue amount at future approved enrollments (subject to employer continuation of the product and payroll deduction availability)
- Accelerated Death Benefit Due to Chronic Illness: Allows access to the whole life benefits during the insured lifetime in the event they are diagnosed with a chronic illness that is expected to be permanent and leaves the insured with the inability to perform 2 of the 6 activities of daily living.
- Accelerated Death Benefit for Terminal Illness: An amount equal to 50 of the face amount of the Certificate.
- Accelerated Death Benefit due to Chronic Illness: No premium payment if insured becomes disabled due to covered injury or sickness and is totally disabled for 6 continuous months.
