Welcome to the 2026 Welch Packaging Employee Digital Benefits Guide! This digital guide offers you and your family members a look into your comprehensive benefits program. We encourage you to take the time to educate yourself about your options and choose the coverage options for you and your family. We have included brief descriptions of our benefit offerings and the cost.

We are committed to providing our employees with a total rewards program that is both comprehensive and competitive. This digital guide provides a general overview of your benefit choices and enrollment information, as well as useful tips, tools, and resources to help you choose the right coverage for you and your family. As you prepare to enroll:

Consider your benefit coverage needs for the upcoming year. For example, is your family financially protected if you can't work due to an accident or illness?

  • Consider other available coverage
  • Gather information you'll need. If you are covering dependents, you will need their dates of birth and Social Security numbers. In addition, you may need to provide documentation verifying their eligibility - such as a marriage license or birth certificate.

Open enrollment is right around the corner! This is your once-a-year opportunity to review your benefits, make changes, and ensure you have the coverage that best fits your needs. Eligibility

Who is eligible for benefits & what is needed

Who's Eligible for Benefits?

Employees:

If you are a regular associate scheduled to work 30 hours or more per week, you are eligible for benefits at the beginning of the next pay period following the completion of the 60th day of employment.

Keep in mind: If you don't enroll for coverage within your first 60 days after your hire date, you will not receive benefits including health, dental, and vision during the plan year, unless you experience a qualified change in family status or enroll in the annual Open Enrollment period.

Dependents:

You may enroll your eligible dependents in many of the same plans you choose for yourself. Eligible dependents include the following:

  • Your legal spouse
  • Your children to the end of the month in which they turn age 26 (including children for whom you are the legal guardian)

Enrolling dependents? Have these items ready:

When you add dependents to your coverage, you must provide the following information:

  • Legal name
  • Date of birth
  • Social Security number (as mandated by the Patient Protection and Affordable Care Act (PPACA)
  • Supporting documentation, such as marriage certificate, birth certificate, adoption papers, and tax documents.

If you do not provide the required information, your dependents may be dropped from coverage.

Working Spouse Rule:

If your spouse is covered under the Welch Packaging Health Plan and has started working or changed employers within the past year, please ask a Human Resource Manager for a form to update their coverage status.

As health insurance costs continue to rise, Welch Packaging must ensure that our costs remain as affordable as possible for everyone. Therefore, the Welch Packaging Health Plan contains a rule that requires spouses who have health insurance coverage available through their own employer to be enrolled in that coverage.

If you wish to cover your spouse on the Welch Packaging Health Plan, you will be required to confirm your spouse does not have coverage available through their employer. if your spouse does not have other coverage available, then your spouse may remain on the Welch Packaging Health Plan.

Your eligible children up to age 26 may remain covered under the Welch Packaging plan, regardless of other available coverage.

The Working Spouse rule does not apply to the Dental Plan or Vision Plan.

Coverage Termination:

Medical, Dental, and Vision
Coverage terms the last date worked
Life and Disability
End of the month following your last day worked
Critical Illness, Accident
End of the month following your last day worked
Dependent Child Turning 26
End of the month following the 26th birthday
Loss of Eligibility
Coverage ends the date that eligibility is not met

Approximately two weeks after you leave Welch Packaging, you will receive a letter from our COBRA administrator regarding options and rates to continue your coverage post termination.

Enrollment Periods:

New Employees:

As a new employee, you are eligible for benefits after a 60-day waiting period.

Open Enrollment:

As a benefits eligible employee, you have the opportunity to enroll in or make changes to your benefit plans during our annual open enrollment period. Open enrollment is usually held in October with benefit elections effective January 1. You may find more information about Open Enrollment here.
